    I understand this is an online course. Each user is responsible for ensuring his/her online system is adequate for accessing the course content.
    I understand that credit for continuing education (CEU) will be provided for physical therapists: All other (non-physical therapy professionals) are responsible for claiming education credit per their professional guideline. Participants understand that all modules and learning activities must be completed in order to receive CEU credit including passing the post-course test with 70% score. All participants are responsible for understanding the continuing education requirements in his/her state and/or setting as applicable to this online course.
    Individuals in states requiring individual course approval (including Texas, Ohio, Nevada, Oklahoma, New Mexico) will be provided the information necessary to complete application for course approval. In addition, individuals in Texas, Ohio, and Nevada will receive a single course discount ($25) OR a multi-course discount ($50) that may be used toward individual course approval application. * Refunded after purchase. NOTE: Please inquire regarding course approval for EI in Illinois.

    CANCELLATION AND REFUND POLICY: Requests for refunds must be submitted in writing to CORE Learning Collective at melanie@corelearningcollective.org within 14 days of purchase. Requests will be processed minus a $15 processing fee. Courses will not be eligible for a refund if the course has been completed and CEUs have been claimed. Course sponsor(s) reserves the right to cancel a course and assumes no responsibility for personal expenses.
